Objectives: To investigate the diagnostic value of fibrinogen (FIB) in patients with rib fractures complicated by lower extremity deep venous thrombosis (DVT). Methods: Analyzing data from 493 patients at Shijiazhuang Third Hospital, FIB levels at 24, 48, and 72 h post-injury were compared between DVT and non-DVT groups. Results: DVT group had elevated FIB levels at all times ( p < .001). FIB at 24 h showed highest AUC, particularly in patients with BMI <28. Conclusion: In conclusion, measuring FIB at 24 h post-injury enhances DVT detection in rib fracture patients, with potential BMI-related variations.

Background The Montreal Cognitive Assessment Scale (MoCA) survey was used to examine the risk factors contributing to the development of mild cognitiveMethods impairment (MCI) in patients with type 2 diabetes mellitus (T2DM) in combination with clinical practice, the Montreal Cognitive, The Assessment Scale (MoCA) was used to assess cognitive function. Based on the MoCA scale scores, subjects were included in a total of 147 cases in the type 2 diabetes mellitus with mild cognitive impairment group (T2DM-MCI group) (MoCA score < 26) and a total of 53 cases in the group with type 2 diabetes mellitus group with normal cognitive function (T2DM-NCI group) (MoCA score ≥ 26 points). While venous serum samples were collected from the patients, the metabolic data were analyzed using ultra-performance liquid chromatography-mass spectrometry (UPLC-Q/TOF-MS) for the T2DM-MCI and T2DM-NCI groups to identify the metabolites with differential expression to analyze their related metabolic pathways between the two groups and to investigate the metabolic characteristics of MCI in T2DM patients.Results The results of comparing general clinical data between the T2DM-MCI group and the T2DM-NCI group showed that there were significant differences in the training and age of the patients.Conclusions Patients with type 2 diabetes mellitus with advanced age, short educational period, high D-dimer level and high glycosylated hemoglobin are more likely to have mild cognitive impairment. Caffeine metabolism and sphingolipid metabolism were the main metabolic pathways, and the abnormalities of their metabolic pathways may lead to the occurrence and development of cognitive dysfunction in T2DM patients.

Objective Dermatomyositis (DM) is an autoimmune disease characterized by chronic muscle inflammation and weakness. Patients with DM are at an increased risk of thromboembolic events (TEs). This study aimed to investigate the prevalence of TEs in DM and to identify the independent predictors. Methods A total of 543 patients hospitalized for DM within the past 10 years were analyzed retrospectively and compared with patients with DM with and without TEs for demographic, clinical, and laboratory characteristics. The independent predictors were analyzed using multivariate logistic regression analysis. The diagnostic performance was calculated by a receiver operating curve (ROC). Results Twenty-two (4.1%) patients with DM had TEs, including 12 (54.5%) with venous thromboembolism and 10 (45.5%) with arterial thromboembolism. Multivariate logistic regression analysis demonstrated that glucocorticoid therapy (odds ratio (OR)=0.003, 95% confidence interval (CI) 0.00–0.03, P <0.001) was a protective factor for the patients with DM developing TEs, whereas increased D-Dimer (OR=1.885, 95% CI 1.21–2.95, P =0.006) was a risk factor. The combined ROC analysis of glucocorticoid therapy and D-Dimer indicated high diagnostic values in distinguishing patients with both DM and TEs from patients without TEs, with 86.4% sensitivity, 98.9% specificity, and 0.983 area under the ROC curve (95% CI 0.962–1.000, P <0.001). Conclusion Patients with DM who have never received glucocorticoid therapy and have increased D-Dimer (>1.3 mg/L fibrinogen equivalent units) should be screened for TEs.
