Diagnostic yield of the lactate stress test in 160 patients with suspected respiratory chain disorder

Abstract: Aim of the present study was to investigate the diagnostic yield of the lactate-stress-test in unselected patients with suspected respiratory-chain disorder (RCD). Serum lactate was determined before, during and after a 15-minute, constant 30W workload on a bicycle in 48 controls and 160 patients with suspected RCD. 122 had definite RCD, 38 neurological disorders other than RCD. The sensitivity, specificity, positive predictive value, negative predictive value and pre-test likelihood of the test was 69, 74, 89… Show more

“…Other tests use an absolute workload, for example 30 W (Finsterer et al 2000). For some authors, for research purposes, it should be performed both a relative and an absolute exercise intensity test (McKenzie et al 2000).…”

“…No unique standardized protocols, at the moment, have been established. The aerobic exercise test can be performed following an incremental cycle test [62][63][64], a cycle test with a fixed constant absolute workload [65][66][67], or a protocol with a constant ''relative'' workload, normalized to the presumed V02max or to the estimated maximal possible work capacity [68,69].…”
