Diagnostics and treatment of insulinoma

Abstract: Insulinomas are the most common functioning pancreatic neuroendocrine neoplasms (pNENs), developed mainly from pancreatic islet cells. More than 90 % of insulinomas are sporadic, benign and small sized. Autonomous production of insulin results in neuroglycopenic and adrenergic symptomatology with potential lethality. Surgery remains the only curative treatment with a high success rate. Preoperative tumor localization is challenging, but important for appropriate surgical approach. Metastatic forms represent a … Show more

“…Insulinoma is currently diagnosed biochemically by measuring plasma glucose, insulin, C‐peptide, and proinsulin during a 12‐ to 72‐hour period of fasting, showing low glucose levels with inappropriately high insulin levels. Treatment options include surgical resection, radiofrequency ablation, alcohol ablation, embolization, chemotherapy, medication, and peptide receptor radionuclide therapy (PRRT) . Since surgical resection is the only curative treatment option for insulinoma, patients are eligible for surgery once the clinical diagnosis has been made and the tumor has been localized using preoperative imaging.…”

“…[ 1 ] Surgical enucleation and/or resection is the treatment of choice for solitary insulinomas. [ 2 3 ] Enucleation is the preferred surgical procedure since it preserves normal pancreatic tissue and is associated with less morbidity. Resection procedures are usually reserved for suspected malignant tumours, tumours abutting the splenic pedicle and multiple tumours.…”

“…Insulinomas, insulin-producing neuroendocrine tumors arising from stem cells or pancreatic β-cells, are the most common cause of endogenous adult hyperinsulinemic hypoglycemia ( 2 ). Persistent hypoglycemia also occurs in neonates and is in most cases caused by congenital hyperinsulinism (CHI).…”
