Dialysis Access, Infections, and Hospitalisations in Unplanned Dialysis Start Patients: Results from the Options Study

Abstract: UPS with initial PD was possible in many patients and was associated with lower requirement for access procedures. AVF formation in UPS patients starting on HD was associated with better 1-year survival. Modality switching in UPS patients requires careful clinical management, including clinical practice patterns promoting permanent HD access formation.

“…In Prol group, six patients had thrombotic events resulting in vascular access failure. On the contrary, no thrombotic events were recorded in the The International Journal of Artificial Organs 43 (1) Max group, while intra-operative and post-operative complications such as significant bleeding hyperflow shunt and steal phenomenon were not observed in both groups. Doppler ultrasound data of the two groups are shown in Table 2.…”

“…AVFs have fewer incidence of complications (such as infection and stenosis), higher patency rates, and longer patient survival compared to arteriovenous graft or central venous catheters. 1 According to National Kidney Foundation’s Kidney Disease Outcomes Quality Initiative (NKF KDOQI) guidelines, the ultrasound features of AVF maturation are defined by the “the Rule of 6”: flow > 600 mL/min, AVF located less than 6 mm from the skin surface, and vein diameter > 6 mm. 2,3 Causes of AVF non-maturation are still a matter of debate.…”

“…Eligible studies included incident chronic dialysis patients (n = 42) 2-4,6,8,9,13-48 and patients with CKD known to nephrology (n = 6). 5,49-53 Study designs included retrospective cohort studies (n = 27), 2,3,5,6,14-16,19-24,26,28,31-36,38,47…”

“…Volume overload and hyperkalaemia are other factors (Brown et al 2015) and acute kidney injury is another (Arrameddy et al 2014). HD is the most common form of dialysis in patients initiating unplanned dialysis (Mendelssohn et al 2011;Brown et al 2015), and the majority, remain on in-centre HD (Machowska et al 2017). Patients who initiate dialysis unplanned have a worse clinical and psychological status, longer hospitalisation and poorer outcomes both in the short and long term (Brown et al 2015) and a higher risk for complications such as infections due to the use of CVC (Miller et al 2016).…”
