Dialysis and Renal Transplantation in HIV-Infected Patients: a European Survey

Abstract: This is the first multinational cross-sectional study of ESRD among European HIV population. Low prevalence of ESRD was found. Two-thirds of patients were excluded from RT for non-HIV/AIDS-related pathologies. Most patients had a functioning graft despite a high acute rejection rate.

“…This has been correlated with higher mortality. [12,[22][23][24] Reasons for incomplete HIV viral suppression have been ascribed to inexperience with HIV treatment and poor prescribing practices by nephrologists, with both under-and over-prescription of ART, and infrequent consultation and follow-up with an infectious diseases specialist. [24] These factors must be considered as possible explanations in this study, as many nephrologists initiate and continue ART without input from an infectious diseases specialist in the medically insured setting.…”

“…Earlier studies, mainly from the USA, concluded that, almost exclusively in Black patients, HIVAN is one of the major causes of ESRD [6,21,22], but histological confirmation of HIVAN is infrequent and the diagnosis is usually made clinically. More recent studies have reported an increasing relevance of traditional risk factors as well as chronic hepatitis C virus (HCV) coinfection in the development of ESRD in HIVinfected patients [7,23].The epidemiology of ESRD outside the USA has not been extensively studied [24][25][26][27] but could provide new insights, as most US cohorts consist of a high proportion of Black patients with coeval injecting drug use (IVDU). Conversely, European cohorts consist of mostly Caucasian HIVinfected patients with various proportions of IDU and far fewer Black patients with almost no IDU [19,26,28].…”

“…In the aforementioned EuroSIDA survey, out of 122 patients with ESRD, 96 received dialysis and 26 renal transplant. The most frequent causes of ESRD were HIV-associated nephropathy and other glomerulonephritis [10]. The advantages and disadvantages of each form of dialysis in HIV-infected patients are presented in Table 1.…”

“…In a cross-sectional multicenter EuroSIDA survey, the prevalence of end-stage renal disease (ESRD) was 0.5% [10], and an observational cohort study conducted in Great Britain showed a 3.8-fold increase in ESRD among the black HIV-positive patients in the cohort during the 12-year study period [11]. The percentage of patients infected with HIV in dialysis centers around the world ranges from 0.3% to as high as 38% [12].…”
