Introduction Any impairment in the hearing ability of a child with cleft lip and palate may cause difficulties in receptive and expressive language. Purpose Check the association between velopharyngeal dysfunction (VPD), compensatory articulation (CA), and peripheral hearing loss in children with cleft palate surgery. Methods Retrospective study with 60 children (group 1: presence of VPD and CA; group 2: absence of VPD, presence of CA; group 3: presence of VDP, absence of CA; group 4: absence of VPD and CA), age 4 to 5 years old, with cleft palate surgery, through the analysis of the hearing, VP, and speech evaluations. Results Group 4 presented 80% normal hearing; group 1 had 60% hearing loss. The conductive hearing loss type was the most frequent. The glottal stop was the most frequent in group 1 and the middorsum palatal plosive in group 2. There was no significant association (p = 0.05) between hearing loss and the presence of compensatory articulations (groups 2 and 4), nor between hearing loss and the presence of VPD (groups 3 and 4; p = 0.12). Statistical significance (p = 0.025) was found when the group with VPD was associated with the group with CA, that is, group 1 with the control group (group 4). Conclusion Significant association between peripheral hearing loss, compensatory articulations, and VPD was verified for the children in group 1, which not only presented compensatory articulations but also VPD.

Home auditory training for children with impairment hearing due to cleft lip and palate

Purpose: to verify the impact of home auditory training performed by parents for 30 days and the contribution to a better performance of the central auditory processing in children with auditory disorders resulting from cleft lip and palate. Methods: case studies were carried out with three children above seven years of age, presented with operated cleft lip and palate. Tests to assess peripheral hearing, behavioral and electrophysiological tests for central hearing assessment and Sustained Attention Auditory Ability ones, before and after the auditory training, were performed. Those responsible for the children answered a questionnaire which verified the auditory and attention behaviors altered by the impairment of the auditory processing. Results: improvement was seen in the majority of the behavioral tests of the auditory processing disorders, and in those which did not show variation after the training, the performance was maintained. Improvement was observed in the sustained attention as well as in the monitoring of all children of the study, post training. Improvement of the auditory behavior and also the decrease of latencies and the increase of amplitude of Na and Pa waves, after the training, were found. Conclusion: there was an enhancement in the performance of the central auditory processing of all children with operated cleft lip and palate, following auditory stimulation.

Purpose of reviewChildren born with a cleft lip and palate (CLP) are at risk of speech problems. There is a high prevalence of conductive hearing loss due to otitis media with effusion and recent research has highlighted broader difficulties with language and educational attainment. There has been increased interest in the impact of auditory processing on a child's development, but little study into links between auditory skills and speech and language development.Recent findingsChildren with CLP consistently struggle with auditory tasks. Difficulties processing words in a noisy background, auditory attention and temporal processing are frequently reported. A recent study in the United Kingdom investigated the auditory behaviours of 95 children aged 5–8 years with nonsyndromic CLP and compared these with speech and language profiles. Auditory difficulties correlated with speech development and most auditory behaviours correlated with language outcomes. There was also a correlation of language with speech outcomes, suggesting a more complex and interdependent relationship.SummaryChildren with CLP present with high levels of difficulties with auditory skills. The potential impact of this on broader development needs further research, but clinicians should ensure that auditory skills beyond the hearing test are monitored, and advice given to parents to minimize the impact.
