Dietary and socio-economic factors associated with beriberi in breastfed Lao infants

Abstract: A case-control study was conducted to investigate the dietary and socio-economic factors associated with beriberi in infants attending three public hospitals in Vientiane, Lao PDR. Forty-three breast-feeding infants with a median (range) age of 3 (1-9) months were admitted with beriberi. This was defined as the presence of signs of congestive heart failure or shock in the absence of fever or other signs of sepsis, hypovolaemia or cardiac abnormalities, with rapid clinical improvement following parenteral thiam… Show more

“…Infants in developing countries who are exclusively breast-fed by thiamine-deficient mothers are at risk of developing acute signs and symptoms of thiamine deficiency. Outbreaks of thiamine deficiency have been reported in developing Asian nations resulting from low thiamine content in some specific varieties of rice, extensive washing and discarding of cooking water of rice, dietary taboos and presence of antithiamine factors in the diet [5][6][7].…”

“…Thiamine deficiency is associated with traditional rice preparation methods, including the use of nonparboiled polished rice (from which the thiaminerich bran has been removed) (4), and may be particularly common in nursing mothers and their infants because of in-creased dietary thiamine requirements of pregnancy and lactation (5,6). Although the accepted cause of thiamine deficiency is dietary inadequacy, to our knowledge, pharmacokinetics of oral thiamine absorption have not been studied in Asian populations at risk of beriberi.…”

“…After absorption, thiamine is present in plasma as thiamine and thiamine monophosphate (TMP) 4 . Thiamine diphosphate (TDP), which is a metabolically important vitamer, is present in red blood cells.…”

“…Thiamine deficiency is found in alcoholics and rice-eating populations consuming polished rice. 1 Wernicke encephalopathy, characterized by ataxia, confusional state, and ocular motor abnormalities, is an acute neurologic manifestation of beriberi that can be seen in adults and children. Neurologic manifestations of thiamine deficiency in infants are nonspecific and include encephalopathy, vomiting, nystagmus, ptosis, moaning, and convulsions-infantile encephalitic beriberi (IEBB).…”

“…Imaging can facilitate early diagnosis of IEBB. [1][2][3][4] The infant brain can be imaged with CT, MR imaging, and cranial ultrasonography (cUS). Use of CT for imaging the infant brain is associated with the radiation-related risks due to use of ionizing radiation and iodinated contrast agents.…”
