Diethylstilbestrol: Observations on Its Use in Duchenne's Muscular Dystrophy (DMD)

Abstract: Several observations suggest that the course of muscular dystrophy and other myopathies may be sex-influenced. Clinically, muscular dystrophy is less severe in women than in men ( l ) , and the female carrier of the Duchenne gene suffers little muscle impairment though her serum enzymes may be higher than normal ( 2 ) . Experimentally, dystrophic female mice grow better and live longer than do dystrophic male mice ( 3 ) , and steroid-induced myopathy is less severe in the female than the male mouse (4). “…Recent evidence suggests that estrogen may influence the degree of disruption and postdamage inflammatory response in the skeletal muscle (Schneider et al 1999;Tiidus 2001). Likewise, administration of diethylstilbestrol to DMD boys reduced CK serum levels (Cohen et al 1972;Cohen and Morgan 1976). This work aimed to assess whether gender dimorphism in mdx mice would influence the process of regeneration in skeletal muscles during distinct phases of muscular dystrophy.…”

“…The dosage of DES-DP was approximately equivalent to the daily oral dose of DES (about 300 fLg/kg) given to muscular dystrophy patients. 4 Enzyme analyses. CPK was measured by the method of Rosalki 10 using Calbiochem Stat-Pak preparations.…”

“…lated to our observations of the effects of diethylstilbestrol (DES) when administered orally to subjects with Duchenne's muscular dystrophy (DMD). 4 Its administration is associated with a lowering of the high serum levels of creatine phosphokinase (CPK) and lactate dehydrogenase (LDH) to 1h to 1f3 pretreatment levels. 4 The mechanism of the lowering is not established, but it may be the result of a reduced efflux of enzyme from diseased skeletal muscle.…”

“…use as a postcoital contraceptive [4], and use in the treatment of Duchenne's muscular dystrophy [5], DES was also widely used to prevent abor tion and other complications during preg- nancy [6,7]. Not until 1970 was DES impli cated as a carcinogen after vaginal clear-cell adenocarcinoma was diagnosed in young women whose mothers had been treated with DES during pregnancy [8.9].…”
