Differences in midazolam premedication effects on recovery after short-duration ambulatory anesthesia with propofol or sevoflurane for gynecologic surgery in young patients

Abstract: Background: Anxiolytic premedication requires careful consideration owing to potential side effects including delayed recovery after ambulatory anesthesia. We aimed to compare the effect of midazolam on recovery profiles postoperatively, depending on whether propofol or sevoflurane was the primary anesthetic. Methods: We enrolled 226 patients (age, 18–50 years) undergoing ambulatory gynecologic laparoscopic surgery. Patients were categorized into propof… Show more

“…It may be administered per os or intravenously about 30 min preoperatively. In contrast to former times when oral doses of 7.5 mg midazolam were regularly prescribed [35 & ], rather low doses were reported in recent trials ranging from 0.02 to 0.035 mg/kg (maximum of 2.5-3 mg) intravenously in outpatient gynecological procedures [36] and in ASA I and II patients [37]. Of outmost importance is the timely application of the substances.…”

“…Midazolam administration may be counterproductive to the aims of day surgery anaesthesia, which is where our guideline has first been implemented, although this is contested. However, if the aim is to reduce accidental awareness, it is worthy of consideration ( Gurunathan et al 2020 , Hyunjee et al 2020 ).…”

“…49 No difference in time to discharge from the postanesthesia recovery unit was shown in a subsequent study as well. 50 In conclusion, we are calling for the reevaluation of a clinical practice that has arisen despite a lack of high-quality medical evidence. Each anesthesia practitioner must determine whether a preoperative anxiolytic dose of benzodiazepine is medically appropriate or inappropriate for a given patient.…”
