Differences in perioperative coagulation between Japanese and other ethnic groups undergoing laparoscopic cholecystectomy

Abstract: Whereas clotting factor activation sthenia is common in Europe and North America, thrombocyte function sthenia occurs in Japanese patients. This difference may account for the differing incidences of phlebothrombosis in Japanese and white populations.

“…It has been noted for years that East Asian populations demonstrate a lower incidence of baseline thrombotic complications such as deep vein thrombosis (DVT) and pulmonary embolism (PE) than in Caucasians. 8,9 These observations of epidemiolog-ical differences of coagulation across distinct ethnic populations have arisen from studies investigating risk factors for cardiac and cerebral vascular events. 10 The annual incidence of venous thromboembolism (VTE) in the United States is generally accepted to range from 70 to 120 events per 100,000 people.…”

“…Although no studies have explored this phenomenon in the context of free flap procedures, recognizing differences in response to injury may help explain observed differences in thrombotic and hemorrhagic flap complications between the East and West. 8,18,19 Iwade et al evaluated the coagulation profiles of 27 Japanese patients undergoing elective laparoscopic cholecystectomy. Levels of coagulation factors were measured prior to start of anesthesia (baseline), postoperatively, and 24 hours following the procedure.…”

“…The need for such a low dose of EN in our population is not well explained, but a number of possibilities exist. First, there is the possibility of ethnic differences between our studied population and those reported in the literature [11] . Second, the use of exclusively biocompatible materials in our study may have reduced the dose needed [12] .…”

“…[1][2][3][4][5][6][7] Although similar guidelines do not exist within the microsurgery realm, recent studies-including ours-have shed light on the difference in coagulative responses between East Asian and Westerners and its significance in the propen-sity for thrombotic or hemorrhagic complications. [8][9][10][11][12][13][14][15] We aim to examine this phenomenon in patients undergoing microsurgical breast reconstruction. With the growing popularity of microvascular surgery, minimizing complications has become a priority. While bleeding at both donor and recipient sites can be catastrophic, another devastating complication-flap failure-is reported between 0.3 and 5% in breast free flaps and is almost…”
