Different Types of Suprafascial Courses in Thoracodorsal Artery Skin Perforators

Abstract: The suprafascial courses of skin perforators facilitate the design and thinning procedures of thoracodorsal artery perforator flaps. They improve the survivability of flaps and make thinning procedures more secure and efficient.

“…Although it features a constant vascular supply (Mayo et al, ), preoperative imaging has been advised (Allen et al, ). The principle of an ideal skin perforator is defined of a size larger than 0.5 mm (Lin et al, ; Miller, Potparic, Colen, Sorrell, & Carraway, ). Modern CTA is competent to locate vessels as small as 0.3 mm (Mathes & Neligan, ) and is widely used to locate profunda artery perforators (DeLong et al, ; Satake et al, ).…”

“…The tedious dissection of perforators of varying diameters and locations, requiring prolonged surgery, is another disadvantage of the TDAP flap, whereas MNS-LD flap was able to be raised more safely in a relatively shorter duration with a mean dissection time of 80 min. Depending on all the factors above, fairly high complication rates up to 21 18 and 23% 16 were reported about the TDAP flap in the literature. In our series, this rate was 7% (1 of 14 cases).…”

“…Colour Doppler ultrasonography was performed in all cases by the surgeon before the operation, to ensure the presence of a good perforator. Inclusion of patients with sufficient perforators has been shown to be of importance to prevent unreliable TAP flaps [5,6].…”
