Differential Diagnosis and Management of Central Auditory Processing Disorder and Attention Deficit Hyperactivity Disorder

Abstract: Children diagnosed with attention deficit hyperactivity disorder (ADHD) frequently present difficulties performing tasks that challenge the central auditory nervous system. The relationship between ADHD and central auditory processing disorder (CAPD) is examined from the perspectives of cognitive neuroscience, audiology, and neuropsychology. The accumulating evidence provides a basis for the overlapping clinical profiles yet differentiates CAPD and ADHD as clinically distinct entities. Common and distinctive m… Show more

“…8B). This outcome is consistent with reports of the co-occurrence of APD with ADHD symptoms (Chermak et al, 1998; 1999; 2002). The anterior INS is thought to be connected to a variety of human social emotions such as compassion, empathy, taste, fear, pleasure, and disgust (Lamm & Singer 2010; Mutschler et al, 2009; Ostrowsky et al 2002; Chen et al, 2014) or homeostatic emotions such as pain (Ostrowsky et al, 2002) (Fig.…”

“…Adjustment of individual thresholds for potential underestimation due to elevated false-alarm rate did not alter these results. As indicated above, there have been no studies characterizing the performance of children or adults identified as having a CAPD on a central-masking task; however, individuals with CAPD do show deficits under conditions of contralateral competition (Chermak et al, 1999). Although an abnormal increase in threshold with no change in false-alarm rate would be consistent with the presence of a CAPD, the lack of such a finding does not preclude the presence of a CAPD.…”

“…Deficits in central auditory function are inferred when an individual demonstrates difficulty with auditory information despite normal hearing. A CAPD may occur at any level of the central nervous system and is observed in a variety of developmental and acquired disorders, including developmental reading disability, brain neoplasm, and epilepsy (Chermak et al, 1999). CAPD is often diagnosed using a battery of tasks, which may include (a) electrophysiological measures (brain stem auditory-evoked responses), (b) a variety of tests using speech stimuli in degraded or competing conditions, (c) masking level differences, and (d) pitch pattern sequencing tests (Stecker, 1992).…”

“…We may also question whether auditory attention complaints in ADHD patients are associated with the existence of comorbid disorders, such as, for example, Central Auditory Processing Disorder, having that in many patients with ADHD this comorbidity was present 22 , 23 .…”
