Differential value of intima thickness in ischaemic stroke due to large‐artery atherosclerosis and small‐vessel occlusion

Abstract: Cerebrovascular disease is known to be responsible for significant morbidity and mortality worldwide. According to the 2018 World Health Organization Global Health Observatory data, ischaemic heart disease and stroke were the two leading causes of death globally, accounting for a combined 15.2 million deaths in 2016 (https://www.who.int/gho). The aetiologies of ischaemic stroke are heterogeneous. According to an aetiology-based classification criteria (Trial of Org 10172 in Acute Stroke Treatment, TOAST), stro… Show more

“…Pruissen et al and Jin et al reported higher CIMT values in patients with LAA compared to SVD. 15,30 This is clearly in line with findings from our cohort, as reflected by the univariable, and multivariable logistic regression analysis. In an analysis of the Northern Manhattan study, CIMT was related to the burden of white matter hyperintensities (WMH), independently from vascular risk factors.…”

“…CIMT is a measure of subclinical atherosclerosis and was shown to be associated not only with vascular risk factor burden but also with large vessel atherosclerosis, small vessel disease, and AF in the general population, independently from vascular risk factors. [11][12][13][14][15][16] According to previous work by our group and others, CIMT may be of value to identify patients with cryptogenic strokes at high risk for newly detected AF or atrial cardiopathy who therefore should undergo intensified heart rhythm analyses. 17,18 Furthermore, CIMT is easy to measure in a standardized manner via Duplex ultrasound and is commonly integrated into the diagnostic algorithm following stroke.…”

“…Recent research by Jin et al showed that radial intima thickness plays a significant role in the differential identification of stroke subtypes ( 24 ), indicating the significance of intimal thickness in clinical studies. However, the connection between CIT and the plaque types, which were strongly linked to plaque vulnerability and adverse CV events, was not further explored.…”

“…Lipid deposits form in the subintima of arteries throughout the body during the progression of atherosclerosis. Recent studies have also revealed a relationship between the classic atherosclerotic risk factors and CIT, which has good diagnostic value for ischemic stroke and CAD and is helpful in identifying subtypes of ischemic stroke ( 23 , 24 ). Therefore, employing CIT to track the advancement of CP could have great clinical value.…”
