This report describes a case where a continuous lumbar epidural anesthesia was inserted for an operation in a patient with classical hemophilia which was unknown at the time of catheter insertion. In addition to an exact description of the intervention, the anamnesis and laboratory contraindications for epidural anesthesia are discussed. Furthermore, the necessity of preoperative investigations and their value when an epidural anesthesia is planned are also discussed.

All patients presenting with proximal femoral fractures should receive adequate analgesia within the emergency department even prior to radiographic imaging. Femoral nerve block should be considered as the method of choice. The insertion of a femoral nerve block catheter has the dual advantage of early analgesia permitting repeated clinical examination in addition to continued post-operative pain management. The cumbersome logistics inherent in this technique within the clinical setting limits its practical application. An initial single-shot regional nerve block followed by a systemic post-operative analgesia protocol was considered an appropriate alternative. The execution of safe, consistent and appropriate regional nerve block anaesthesia is reliant on formal guidelines and protocols as agreed by the multidisciplinary teams involved with patient-directed pain management and good clinical practice.

Anaesthesia today is a cross-over of all medical sub-specialities, based on a stable scientific fundament and a high margin of safety. Anaesthetic techniques are sophisticated and enable a differentiated treatment of pre-, intra- and post-operative medical demands. The physician-patient-relationship and the patients' satisfaction are main aspects of peri-operative care by anaesthetists even in the area of conflict by economic pressure. Nonetheless, the credit to anaesthetists given by physicians and also by the general public is inadequate resulting in difficulties to recruit a sufficient percentage of the decreasing number of aspiring physicians. This could be prevented by an improved medical school and training. Anaesthesiologists in science address either fundamental questions as to mechanisms of anaesthesia and epidemiological aspects of peri-operative medicine.
