Differentiation between spinal cord diffuse midline glioma with histone H3 K27M mutation and wild type: comparative magnetic resonance imaging

Abstract: Purpose Diffuse midline glioma with histone H3 K27M mutation is a new entity described in the 2016 update of the World Health Organization Classification of Tumors of the Central Nervous System. The purpose of this study was to evaluate the clinical and imaging characteristics to predict the presence of H3 K27M mutation in spinal cord glioma using a machine learningbased classification model. Methods A total of 41 spinal cord glioma patients consisting of 24 H3 K27M mutants and 17 wild types were enrolled in t… Show more

“…Additionally, the boundaries of the tumor are ill-defined, and mild peritumoral edema may be notable. These radiological features are consistent with a high-grade glioma (4,11,12). Some scholars summarized that H3 K27M-mutant DMGs occurring in the cervical spinal cord usually showed uniform enhancement (13).…”

“…Regarding its specificity and sensitivity, they are calculated to be at least 88.2% and 95% respectively. 14 It is now deemed 15 that aggressive surgical excision is the most effective therapeutic strategy for most spinal cord gliomas. With constant advances in imaging techniques and increasingly clear operating microscopes, an increasing number of tumors can be detected at an early stage.…”

“…While recent advancements in the understanding of the pathology of diffuse midline gliomas and identification of targetable pathways have been dependent on tissue sampling, groups are currently investigating noninvasive methods for evaluating mutational status in these tumors via imaging [45,46] or sequencing of circulating tumor DNA [47,48]. However, biopsy remains the gold standard for clinical trial consideration.…”
