Differenzierung von malignen und nichtmalignen, inflammatorischen Perikardergüssen mit Biomarkern

Abstract: Disorders of the pericardium are commonly associated with pericardial effusion. Its etiology comprises a broad spectrum of diseases including also malignancies. Pericardiocentesis, pericardioscopy and targeted epicardial biopsy with consecutive pericardial fluid and epicardial biopsy analysis by cytology, molecular biology and immunology establish the underlying etiology in the majority of cases. Of particular therapeutic and prognostic importance is the definite differentiation of malignant pericardial effusi… Show more

“…Metastatic tumors, especially lung and breast cancer, esophageal squamous cell carcinoma, malignant melanoma, thymic carcinoma, kidney, bladder malignancies as well as mesenchymal tumors like Ewing's sarcoma or hematological malignancies (mostly leukemia, non-Hodgkin's lymphoma and Hodgkin's lymphoma) are the most common causes for pathological PE (7) . Primary tumors of the pericardium such as mesothelioma, fibrosarcoma, lymphangioma, hemangioma, teratoma, neurofibroma and lipomas are rare (8) . The third most common cause is infection and it is often viral.…”

“…Assessment of left-ventricular systolic function, myocardial perfusion imaging to detect ischemic complications, as well as cardiac biomarkers like troponin I, and T, B-type natriuretic peptide might also be considered in selected patient populations at high risk [35,36,38,44]. These investigations will be useful to anticipate cardio-specific treatment and decide on the most appropriate pharmacological therapy for congestive heart failure at early onset [26].…”
