Diffuse Large B-cell Lymphoma Diagnosed by Intracardiac Echocardiography-guided Cardiac Tumor Biopsy

Abstract: A 44-year-old man presented with exertional dyspnea. Transthoracic echocardiography (TTE) revealed a large tumor protruding into the right atrium and extending into the left ventricle. Cardiac magnetic resonance imaging and contrast enhanced computed tomography also confirmed the intracardiac tumor detected by TTE. An endomyocardial biopsy was performed under the intracardiac echocardiography (ICE) guidance, and he was diagnosed to have diffuse large B-cell lymphoma following the histological analysis. ICE-gui… Show more

“…ICE is known to give compatible or superior imaging quality to TEE in guiding transcatheter endomyocardial biopsy without the need for general anesthesia. 21) Although several case reports and the present case showed the usefulness and safety of ICE in differential diagnosis of cardiac masses including malignant tumors, [22][23][24][25] the diagnostic accuracy and complication rate of ICE-guided percutaneous transcatheter biopsy for cardiac masses need to be investigated.…”

“…In addition to a report of cardiac tumors that were diagnosed by a PTB in the right ventricle, primary cardiac lymphoma diagnosed by a PTB in the right atrium has been reported. [ 6 7 ] We described a case of primary cardiac angiosarcoma that was diagnosed by PTB in the right atrium.…”

“…Transvenous endomyocardial biopsy under TEE guidance is useful, simple, and safe [ 22 – 25 ] despite usually requiring local pharyngeal or general anesthesia. In contrast, the recently introduced procedure of transvenous biopsy via cardiac catheterization with the guidance of intracardiac echo-cardiography (ICE) does not require such anesthesia and can facilitate assessment of cardiac structure [ 26 – 30 ]. Physicians who are involved in diagnosing cardiac tumors should be well versed in use of ICE.…”
