Xanthomas are a common manifestation of lipid metabolism disorders. They include hyperlipemic xanthoma, normolipemic xanthoma, and a related condition, necrobiotic xanthogranuloma (NXG). All 3 forms can be associated with monoclonal immunoglobulin (MIg). In an attempt to improve diagnosis, understanding, and treatment of this association, we retrospectively analyzed a personal series of 24 patients (2 hyperlipemic xanthoma, 11 normolipemic xanthoma, and 11 NXG) and 230 well-documented reports from the literature. With the exception of the nodules and plaques featured in NXG, the clinical presentation of xanthomatous lesions usually resembled that seen in common hyperlipidemic forms and could not be used to suspect MIg-associated xanthomas. Extracutaneous sites were not rare. The MIg was an IgG in 80% of cases. Myeloma was diagnosed in 35%. Hypocomplementemia with low C4 fraction was present in 80% of studied patients. ÖzetKeratinizasyon bozukluklarının önemli bölümü konjenital hastalıklar olmakla birlikte edinsel olarak ortaya çıkan keratinizasyon bozuklukları da vardır. Bunların bir çok nedeni arasında sistemik maliniteler de yer alır. Edinsel iktiyoz, Bazex sendromu ve "tripe palms" (işkembe avuç içi) paraneoplastik ilişki oranının yüksek olduğu hastalıklar arasında yer alır. Akantozis nigrikans ise malinite ile birlikte olduğunda çok şiddetli seyreder ve özel klinik görünümlere yol açar. Başka bir nedenle açıklanamayan edinsel palmoplantar keratoderma nadir de olsa paraneoplastik olabilir. Bu hastalıklara erken ve doğru tanı konması malinitelerin erken saptanmasını sağlaması açısından önem taşır. Among their multiple causes systemic malignancies deserve special attention. Acquired ichthyosis, Bazex syndrome and tripe palms are among diseases with highest ratio of paraneoplastic association. Ein 35-jähriger Mann südamerikanischer Herkunft stellte sich mit seit 5 Monaten progredienten Hautveränderungen im Nacken, an Händen und Füßen sowie in den Beugen vor. Es zeigten sich plantar fokale Hyperkeratosen (Abbildung 1) mit Transgredienz auf die Dorsalseiten der Zehen und die laterale Fußkante, an knuckle pads erinnernde graubraune Hyperkeratosen über den dorsalen Interphalangealgelenken beider Hände (Abbildung 2a), spitzkegelige Papeln an den distalen Dorsalseiten der Finger (Abbildung 2b) sowie eine diskrete fokale palmare Hyperkeratose. In den Ellenbeugen, Axillen (Abbildung 2c), Kniebeugen sowie im Nacken (Abbildung 2d) imponierten gräulich-schwärzliche, feste Hyperkeratosen mit Vergröberung des Hautreliefs.
