Diffuse Pneumocephalus : A Rare Complication of Spinal Surgery

Abstract: J Korean Neurosurg Soc 48 : [288][289][290] 2010 The common etiologies of pneumocephalus, presence of air in the intracranial cavity, are trauma and cranial surgery. Pneumocephalus after spinal surgery is an unusual postoperative complication. We report the case of a male 59-year-old man who developed a pneumacephalus after posterior lumbar surgery for spinal stenosis. Intraoperatively, a cerebrospinal fluid leak following a dural tear was noted and immediately repaired. The next day, the patient complained… Show more

“…We searched PubMed and found some case reports of pneumocephalus/pneumorrhachis due to a CSF fistula that was associated with surgery. Various types of surgery were reported, but majority were lung resection 3,5,6) and the spinal surgery 4,7,9,11) . In almost all reported cases, the intervals between surgery and the development of pneumocephalus/pneumorrhachis were relatively short, from a few days 5,7,9,11) to a few months 3,4) , but the few cases of several years after the surgery were also reported 6,10) .…”

“…Therefore, we considered the well-known etiology of spontaneous pneumocephalus, which has several suggested mechanisms for its development 10,11) . One potential mechanism is that air enters the intracranial cavity through a defect in the skull (known as the ball valve mechanism).…”

“…A small amount of the air is asymptomatic and misleading and resolves spontaneously after a few days, however, high inspired oxygen concentration can accelerated its absorption (11). Surgical intervention in order to repair dura mater defects should only be performed when there is neurological deficit, persistent CSF leakage or infection (1,12). Simple X ray, CT scan or MRI diagnose the air in spinal and cranial space.…”

“…Pneumocephalus is commonly established after neurosurgical procedure due to iatrogenic spinal dura tear, but it can also be the result of trauma (1). Post traumatic fractures of temporal bone air cells or paranasal sinuses or skull base can cause dura lacerations and pneumocephalus (1,2).…”
