Background: Androgenetic alopecia (AGA) is a common hair disorder which affects the patients' psychology and social activities. Despite being the most prevalent alopecia in Malaysia, there are limited studies examining its psychological impact and patient's perception on AGA. This study assessed these effects and determined their contributing factors in AGA patients. Methods: A prospective observational study was conducted involving all AGA patients attending dermatology clinics at two tertiary centers. Psychological impact was assessed using Hospital Anxiety and Depression Scale (HADS). Patient's perception was assessed using Visual Analog Scale (VAS). Results: Sixty male patients and 41 female patients were recruited. The median age of patients was 29 years. The ethnic composition was Malay (30.7%), Chinese (34.7%) and Indian (34.6%). More females experienced mild AGA-related anxiety and depression compared to males (P<0.001). Young (age<40 years) and single individuals experienced more anxiety and depression than older, married patients (P<0.001). Patients who adopted coping strategies reported more anxiety (P=0.002) and depression (P<0.001) compared to those who were not using any coping strategy. Women were more dissatisfied with their hair condition (VAS score=4 [3])) compared to men (VAS score=5 [3]; P=0.002). Conclusion: AGA impairs patients' psychology and affects their self-perception. Age less than 40 years old, female, unmarried, disease duration less than 4 years and adopting coping strategies were significant factors influencing the psychological impact and perception in AGA patients. Hence, early integration of psychological support to treatment of AGA is recommended.

Objective: Psoriasis is one of the most common chronic skin diseases, which has a negative impact on the interpersonal relationship and psychosocial well-being. Therefore, psoriasis may lead to a decrease in the self-esteem of the patients. Increased level of anger often accompanies patients with psoriasis. Our aim is to investigate the relationship of anger, anger expression style and level of self-esteem in patients with psoriasis and to determine whether duration and severity of disease affects anger, anger expression style and level of self-esteem. In addition, we aimed to compare the level of self-esteem in patients with early and late onset of psoriasis. Methods: Eighty-five patients with psoriasis and 86 healthy controls were included in the study. Severity of disease was calculated with Psoriasis Area and Severity Index (PASI). The patients were classified as early-onset (age < 20 years) and late-onset psoriasis (age ≥ 20 years). Duration of disease and socio-demographic characteristics were recorded. State-Trait Expression Inventory for Anger (STAXI) and Roserberg Self-esteem Scale (RSES) were used for determining anger, anger expression style and self-esteem. Results: Trait anger, state anger and anger-in scores were statistically significantly higher in patients with psoriasis (p < .05). Anger-out and anger-control scores were similar in both groups. RSES scores were statistically significantly higher in the psoriasis group (p < .05). There was a negative weak statistically significant correlation between RSES and angercontrol scores (r = −0.246, p = .027). A positive, weak, statistically significant correlation was found between RSES scores and anger-out scores (r = 0.224, p = .045). A positive, mild, statistically significant correlation between duration of the disease and anger-in scores (r = 0.277, p = .027) was detected in patients with psoriasis whereas no statistically significant correlation between the other parameters and duration and severity of the disease was detected. No significant difference was detected when patients with early-and late-onset psoriasis were compared in terms of self-esteem (p = .722). A positive, mild, statistically significant correlation between duration of the disease and anger-in scores (r = 0.277, p = .027) was detected in patients with psoriasis whereas no statistically significant correlation between the other parameters and duration and severity of the disease was detected. Conclusion: Reduced self-esteem and increased anger levels are remarkable in psoriasis patients. While evaluating and arranging treatment of psoriasis patients, it should be considered that psoriasis is not only a dermatological disease, but also a disease resulting in reduced self-esteem and increased anger level; therefore dermatologic and psychiatric approaches should be taken with the patients. ÖzAmaç: Saç ve saçlı deri hastalıkları dermatoloji poliklinik başvurusunun önemli nedenlerindendir. Bu çalışmanın amacı Türk kadınlarında görülen saç ve saçlı deri hastalıklarının değerlendirilmesidir. Yöntemler: Şubat 2015 ile Şubat 2018 yılları arasında deri ve zührevi hastalıkları polikliniğine başvuran 18 yaş üstü 16743 kadın hastaya ait dosya retrospektif olarak tarandı. Verilerde eksiklik bulunmayan 1857 kadın hasta çalışmaya dahil edildi.Sonuç: Kadınlarda dermatoloji polikliniğine başvuru nedenleri içerisinde saç ve saçlı deri hastalığının oranı %11,09 idi. Skatrisyel alopesi ile sonuçlanan saçlı deri hastalıkları oranı ise düşüktü. Saç güzellik algısının bir parçası olduğundan, saç ve saçlı deri hastalıklarının erken tanı ve tedavisi, kişilerin özgüven ve yaşam kalitesine etkisinden dolayı önemlidir. Anahtar kelimeler: Saç, saçlı deri, kadın, saç hastalıkları, saçlı deri hastalıkları. AbstractObjective: Hair and scalp diseases are an important reason for admitting to dermatology clinics. The aim of this study is to evaluate the hair and scalp diseases in Turkish women.Methods: The records of 16743 female patients above 18 years of age, who were admitted to dermatology outpatient clinic between February 2015 and February 2018, were screened. One thousand eight hundred and fifty seven female patients having hair and scalp diseases, without any missing information, were included in the study.Results: One thousand eight hundred and fifty seven female patients (%11.09) had hair and scalp diseases. The mean age of patients were 35.82±15.23 years (18-94 years). One thousand three hundred and seventy three patients (73.9%) were under 45 years of age; 484 patients were above 45 years of age.There were 12 types of hair and scalp diseases. The total number of diseases was 1931 and, there were two types of diseases in 73 patients (3.9%) and three types in one patient (0.1%). Disease rate that cause the cicatricial alopecia was %2.27. Seborrheic dermatitis, telogen effluvium, psoriasis, alopecia areata, and androgenic alopecia were the most common diseases (43.5%, 25.4%, 16.4%, 6.7%, and 4.8%, respectively). When diseases were evaluated according to age groups (<45 years and ≥45 years), the most common disease were seborrheic dermatitis in both groups, followed by telogen effluvium and psoriasis in group of under 45 years of age; psoriasis and telogen effluvium in group of above 45 years of age. Fourteen female patients had pediculosis. The most common scalp diseases were seborrheic dermatitis and psoriasis; hair diseases were telogen effluvium and alopecia areata. The most common associated diseases were seborrheic dermatitis and telogen effluvium.Conclusion: The rate of hair and scalp diseases among women admitted to the dermatology clinic was 11.09%. However, the rate of the scalp diseases resulted in cicatrial alopecia were low. Hair is a component of beauty perception, so earyl diagnosis and treatment of hair and scalp diseases are important for impact on both self-esteem and quality of life.
