Discharge home after acute stroke: Differences between older and younger patients

Abstract: The presence of a spouse is an additional important factor determining discharge home in older stroke patients. It is important to monitor and support informal caregivers in order to provide appropriate care for older community-dwelling stroke patients.

“…Other studies have investigated the role of discharge disposition on readmission rates among patients hospitalised for other conditions . However, this is the first study to demonstrate a relationship between discharge dispositon and readmission rates within a sample of patients hospitalised for CAP, among whom discharge home with HHC conferred a 58% greater risk‐adjusted likelihood of 30‐day readmission.…”

“…However, few studies have rigorously investigated the relationship of discharge disposition and the risk of readmission in CAP patients. In particular, while discharge disposition has been linked to readmission among patients admitted for other conditions, such as hip fractures, dementia, chronic obstructive pulmonary disease (COPD) and stroke, there is no literature on this relationship among patients hospitalised for CAP …”

“…The positive association between lower age and participation confirms the importance of age in the prediction of long-term participation restrictions. Furthermore, despite the importance of having a spouse for determining discharge destination was highlighted by a recent study [43], the presence or absence of a spouse was not associated with favorable or unfavorable participation one year after stroke.…”

“…Each year approximately 795,000 people in the United States experience strokes, of which eighty per cent are ischaemic strokes (Mozaffarian et al, 2015). Nearly half, if not more, of all ischaemic stroke survivors (ISSs) are discharged from hospital directly to home (Dutrieux et al, 2016;Mozaffarian et al, 2015). ISSs suffer from fatigue and difficulty communicating, adverse psychological reactions (e.g., anxiety and stress) and/or impaired cognitive function (e.g., inability to focus on multiple tasks) (Nadarajah & Goh, 2015;Winstein et al, 2016).…”
