Discordance in the diagnostic assessment of vulnerable plaques between radiofrequency intravascular ultrasound versus optical coherence tomography among patients with acute myocardial infarction: insights from the IBIS-4 study

Abstract: We aimed to evaluate the diagnostic agreement between radiofrequency (RF) intravascular ultrasound (IVUS) and optical coherence tomography (OCT) for thin-cap fibroatheroma (TCFA) in non-infarct-related coronary arteries (non-IRA) in patients with ST-segment elevation myocardial infarction (STEMI). In the Integrated Biomarker Imaging Study (IBIS-4), 103 STEMI patients underwent OCT and RF-IVUS imaging of non-IRA after successful primary percutaneous coronary intervention and at 13-month follow-up. A coronary le… Show more

“…Similarly, in the study by Fujii et al, the combined use of IVUS and OCT improved TCFA detection accuracy [ 69 ]. However, a high discordance rate (70.3%) was observed between IVUS and OCT in the recognition of TCFAs in a recent study, perhaps attributable to a VH-IVUS-based relevant false-positive rate [ 70 ]. In summary, these tools provide complementary and additive information: while OCT is the gold standard for assessing plaque microstructures and measuring FC thickness, VH-IVUS is superior in the assessment of PB.…”

“…All OCT assed TCFA ( n = 14) were confirmed as TCFA by RF-IVUS. The concordance rate between RF-IVUS and OCT for TCFA diagnosis was 29.7% [ 45 ].…”
