Discriminatory power of MRI for differentiation of adrenal non-adenomas vs adenomas evaluated by means of ROC analysis: Can biopsy be obviated?

Abstract: The purpose of our study was to evaluate the discriminatory power of MRI in high-field magnet (1.5 T) for differentiation of adrenal non-adenomas vs adenomas assessing the following parameters separately and in combination: mean diameter of adrenal mass; previously described and new ratios as well as index calculated from signal intensity (SI) on SE T2-weighted images, chemical shift imaging (CSI), and Gd-DTPA-enhanced dynamic studies. One hundred eight adrenal masses (36 non-hyperfunctioning adenomas, 27 pheo… Show more

“…5). Using the SI index (SII) signal loss greater than 1-16.5% have been reported [27,30,32]. A 100% accuracy for differentiating adenomas and non-adenomas has been reported in separate studies by Tsushima et al and [27] Fujiyoshi et al [30] using SII threshold values of >5% and >16.5%, respectively.…”

“…Ichikawa et al [39] berichten über eine Sensitivität von 92% und eine Spezifität von 95% bei Anwendung dieses Kriteriums. Die CSI scheint bisher von allen untersuchten MRT-Techniken am genauesten zu sein (Abbildung 1) [37,[40][41][42].…”

“…Magnetic resonance imaging (MRI) using in-phase and opposed-phase images, known as chemical-shift MRI, has been proposed as an effective alternative to CT for characterization of adrenal masses. (6,7). Quantitative analysis of the signal intensity of adrenal lesions compared with that of the liver, fat, muscle and image background was subsequently shown to improve characterization of adrenal masses compared with qualitative analysis of signal intensity variations.…”
