Disease severity and status in Stevens–Johnson syndrome and toxic epidermal necrolysis: Key knowledge gaps and research needs

Abstract: Stevens–Johnson syndrome and toxic epidermal necrolysis (SJS/TEN) are on a spectrum of cutaneous drug reactions characterized by pan-epidermal necrosis with SJS affecting < 10% of body surface area (BSA), TEN > 30%, and SJS/TEN overlap between 10 and 30%. Severity-of-illness score for toxic epidermal necrolysis (SCORTEN) is a validated tool to predict mortality rates based on age, heart rate, BSA, malignancy and serum urea, bicarbonate, and glucose. Despite improved understanding, SJS/TEN mortali… Show more

“…The purpose of this report is to provide a first step in understanding the severe immune TEN reactions to oritavancin. Traditionally, treatment in SJS/TENs patients consists of supportive measures including fluid resuscitation, hemodynamic stabilization, wound care, and pain management until the affected skin is reepithelialized [15]. In this patient case, intubation was elected to protect the airways due to oral involvement.…”

“…15 Based on the BSA of skin detachment, SJS affects <10%, SJS/TEN overlap 10-30% and TEN affects >30% of BSA. 16 A total of 4268 citations were searched in PubMed/Medline database for afatinib associated SJS/TEN and only 5 cases were described (Table 1). 7,[17][18][19][20] The latency periods from drug introduction to appearance of skin lesions were 5-64 days.…”

“…SJS/TEN is an immune‐mediated reaction in which cytotoxic T cells (CD8+) target keratinocytes and induce full‐thickness necrosis of the epithelial layer 15 . Based on the BSA of skin detachment, SJS affects <10%, SJS/TEN overlap 10–30% and TEN affects >30% of BSA 16 …”
