Disorders of puberty

Abstract: Over the past 20 years, a clear secular trend toward the earlier onset of puberty has been described. A better knowledge should help clinicians attempting to define both precocious and delayed puberty (PP and DP, respectively). The definition of PP for girls is the appearance of secondary sex characteristics development before the age of 8 years, while DP is based on the absence of thelarche at the age of 13 years. Regarding PP, one should clinically distinguish between true precocious puberty, i.e., complete … Show more

“…During gonadarche (Figure ), the hypothalamic‐pituitary‐gonadal (HPG) axis is activated, and several hormones have been identified to participate in the activation of the HPG axis including kisspeptin, neurokinin B, dynorphin, leptin, and ghrelin . Kisspeptin, neurokinin B, and dynorphin are released by specialized neurons, the KNDy neurons in the hypothalamus . Kisspeptin is a key regulator of the pulsatile secretion of gonadotropin releasing hormone (GnRH) from the hypothalamus .…”

“…For this reason, clinical assessment of gonadal dysfunction should not be postponed to the long-term follow clinic (i.e., 5-year survival), but should start in children as they approach the normal age of pubertal onset, which is around 11 years in girls and around 12 years in boys in European countries. In the US and other countries, a downward trend is observed with regard to onset of puberty, implying assessment may be started earlier [4, 5]. …”

“…Precocious puberty is a disease associated with puberty development, which affects the physical and mental health of children [13], causes the incidence rate of obesity to increase, and increases the risk of cardiovascular disease and metabolic diseases in children after adulthood [14]. e etiology of some children with precocious puberty is not clear, and about 15% of children with precocious puberty can develop into central precocious puberty by inducing the rapid maturation of hypothalamus pituitary gonad axis [15,16].…”
