Distal Oblique First Metatarsal Osteotomy in Grade 3 Hallux Rigidus: A Long-Term Followup

Abstract: The oblique osteotomy is a safe and reliable procedure for treatment of painful hallux rigidus. In our experience, it yields good and excellent results with high patient satisfaction and low complication rates.

“…Although etiology and biomechanics of this problem remain poorly understood, reports performing osteotomies on the first metatarsal that pursue declination of the metatarsal head have demonstrated satisfactory functional outcomes [2,3]. However, the precise declination produced by the osteotomy continues to be subjectively estimated, primarily due to the scant objective parameters to quantify it.…”

“…Decompressive osteotomies of the first metatarsal have had reported success rates in the literature ranging from 54% to 95%. 1,[15][16][17] Our satisfaction rate of 85.2% falls within this range but is higher than the 73.3% reported by Roukis in a large systematic review. 15 Our satisfaction rate overall for the cheilectomy group was 80.0%, which is lower than rates reported in other published studies.…”

“…It is worth highlighting the study by Roukis,26 consisting in a systematic review of 93 cases in which 22.6% required revision and 30% postoperative metatarsalgia. Nevertheless, Malerba et al 27 published very good results with the implementation of the modified Weil-type osteotomy for the first metatarsal (95% of patients with good or excellent results and only 1 case of postoperative metatarsalgia in a series of 20 patients with grade III H. rigidus).…”
