Distal radius triplane fracture

Abstract: A triplane fracture is so named because of the three planes traversed by the fracture line. These are physeal fractures that result from injury during the final phase of maturation and cessation of growth. This fracture pattern typically involves the distal tibia. We present a rare case of a triplane fracture involving the distal radius.

“…Typically seen in adults, intra-articular fractures of the distal radius are high-energy injuries caused by impaction of the carpus into the distal radius, often resulting in a dorsomedial fragment known as a die-punch fragment [14,15]. Although intra-articular fractures of the distal radius are rare in children [13], these injuries have the potential to cause premature physeal arrest, late deformity, impaired wrist or forearm motion, ulnocarpal impingement, or disruption of the triangular fibrocartilage complex [16][17][18][19][20].…”

“…However, locations other than the distal tibia are very rare and only a few individual case reports are documented [18][19][20][21]. Until today only five case reports of transitional fractures of the distal radius have been published [11][12][13][14][15][16].…”

“…Transitional fractures are defined as epiphyseal injuries in which partial closure of the epiphyseal growth plate has already occurred [10] and must be clearly differentiated from pediatric fractures as an independent fracture entity. Despite the frequency of distal radius fractures, especially during childhood, transitional fractures of the distal radius are rare and up until today only a few case reports have been published [11][12][13][14][15][16]. A case series of four transitional fractures of the distal radius with discussion of the diagnosis and treatment options and critical review of the literature is presented here.…”
