Distal Tibial Epiphyseal Fracture (Tillaux) and Capsular Interposition

Abstract: Arthroscopic-assisted reduction is one recommended technique for pediatric distal tibial epiphyseal fractures. A 15-year-old male patient with a triplane distal tibial epiphyseal fracture (Tillaux type) was scheduled for arthroscopic reduction and pinning, which failed to provide adequate fracture reduction. Open surgery was subsequently performed, which revealed a capsular interposition in the fracture line attributable to an anterior capsular tear of the ankle. The possibility of capsular interposition shoul… Show more

“…The integrity of the anterior tibiofibular ligament is important for stabilization of the ankle. An early diagnosis plays a significant role in successful treatment and recovery (12). The fragments of Tillaux-Chaput fracture are smaller and result in slight swelling, deformity, and obstruction of the fibula, with the result that these fractures tend to be misdiagnosed as a simple ankle sprain (13)(14)(15).…”

“…Reduction is usually achieved with internal rotation. For fractures with >2 mm of residual displacement measured by CT scan following an attempted closed reduction, anterior capsular interposition should be considered 69 . Open reduction and removal of the capsule may be necessary, and a single screw parallel to the physis is usually sufficient for fixation (Fig.…”

“…However, there was no consensus between the individual cases that the complementary radio-diagnostic examination was most appropriate. The critical point was that an early diagnosis played a signifi cant role in a successful treatment and recovery (8). Conservative treatment consisted in the mentioned cases of fi xation for six weeks, by long leg cast with the internally rotated foot (13) without allowed load of the injured limb.…”
