Distal Triceps Rupture Repair: The Triceps Pulley-Pullover Technique

Abstract: Distal triceps rupture is an uncommon but debilitating injury, and surgical fixation is almost invariably warranted. A number of techniques have been described in the literature in which combinations of transosseous tunnels and bone anchors have been used. We describe a modification to existing techniques—the triceps pulley-pullover technique with all-suture anchors. This technique minimizes bone loss, while maximizing the bone-tendon contact area and creating a double-row repair to optimize strength and heali… Show more

“…Current smallscale studies report rerupture and consequent revision rates between 0% and 25% in high-demand cohorts, which can vary based on the surgical technique, although most reported revision rates are between 0% and 9.5%. 4,20,[24][25][26] Anabolic steroid use is thought to be a risk factor for distal triceps tears. 2,5,6,23 For our study, patients with a prescription for anabolic steroids within 1 year before the surgery had a statistically significant increase in revision rates.…”

“…It has also been described that multiple suture knots in this region might cause inflammation in the soft tissue envelope. 4,6 A cadaver study by Carpenter et al 3 adjusted these techniques by using an equal number of sutures in both, and providing a tendon-compressing effect in the transosseous technique similar to the knotless double-row repair. No significant difference in tendon displacement or footprint coverage was found after cyclic-loading, contrasting with previous studies.…”

“…Surgical treatment is the standard of care for triceps injuries in active individuals. Options for repair include transosseous, 1 , 2 , 3 , 4 , 5 , 6 , 7 suture anchor (SA), 1 , 2 , 4 , 8 , 9 and hybrid transosseous-SA techniques. 3 , 5 , 6 , 10 The transosseous cruciate (TC) technique has been the historical standard for treatment of these injuries.…”
