Distinct Recurrence Pattern and Outcome of Adenocarcinoma of the Gastric Cardia in Comparison with Carcinoma of Other Regions of the Stomach

Abstract: Our data indicate that the prognosis of patients with adenocarcinoma of the gastric cardia is extremely poor. To improve their prognosis, new treatments in addition to gastrectomy with extensive lymph node dissection are needed.

“…This difference in survival is confirmed by other studies, although prognosis in the East is still far better with a 5-year survival of 62%. 4,24 Gastric carcinoma can be divided into two distinct histological patterns, a diffuse and an intestinal type according to the Laurè n classification. 25 In the last decades, a rise in the incidence of diffuse type of carcinoma is seen worldwide.…”

“…A statistically significant finding could be noted in intestinal-type but not in sporadic diffuse-type cancer (GG-plus-GA vs. AA). As is known, cardia-type gastric cancer differs from noncardia-type gastric cancer in etiology, pathology, carcinogenesis, and/or prognosis (Heidl et al, 1993;Kim et al, 2005;Saito et al, 2006), so is intestinal-type cancer versus sporadic diffuse-type cancer. It could be said that the indiscriminate combination of cardia-type and noncardia-type cases or intestinal-type and diffuse-type cases in the majority of eligible studies may mask or at least underestimate the strength of the real associations (Xue et al, 2010Cui et al, 2011).…”

“…Though we cannot explain the differences, the prognosis of tumors in the lower third of stomach is better than that in any other locations of stomach [16][17][18] . We presume that the prognosis of gastric cancer is associated with the metastasis of gastric cancer cells into the vagina vasorum.…”
