Distinction of Risk Factors for Superficial vs Organ-Space Surgical Site Infections After Pancreatic Surgery

Abstract: Preoperative biliary stenting and coriticosteroid use increase superficial SSI, even in patients receiving perioperative piperacillin-tazobactam. Additional measures, including extended broad-spectrum perioperative antibiotic treatment, should be considered in these patients. Organ/space SSIs appear to be related to pancreatic fistulae, which are not modifiable. Reporting these different subtypes as a single, overall rate may be misleading.

“…We also acknowledge the stringent nature of the eligibility criteria and its limits on the trial's generalizability to a higher-risk patient cohort. Discharge by POD 5 after PD is radically below the national median, 40 and these criteria were conceived to ensure safety. These data appear applicable to high-performing PD patients at low risk of complications and suggest that if these patients are recovering well from operations, hospitalization beyond POD 5 is unnecessary.…”

“…Surgical site infection of Japan Clinical Oncology Group postoperative complications criteria grade 2 or higher was defined as the primary outcome, which was characterized by infection of the incision or intra-abdominal space requiring pharmacological or surgical treatments [22]. We also investigated the incidence of postoperative pancreatic fistula, a postoperative complication associated with space surgical site infections in pancreatic surgery [13,24]. A postoperative pancreatic fistula was defined according to the International Study Group on Pancreatic Fistula guidelines [25].…”

“…Surgical site infections (SSI) were classified in a simplified manner as [ 8 , 9 ] parietal abscesses or intra-abdominal collection. A respiratory infection was defined as a fever associated with a biological inflammatory syndrome and imaging suggestive of bronchopulmonary infection.…”

“…They will also have oncological impact, delaying the start of adjuvant treatment and doubling the risk of not receiving it [ 5 , 6 , 7 ]. There are two types of infectious complications, surgical site infections (SSI), as defined by the Center for Disease Control [ 8 , 9 ], and general infectious complications, such as urinary, respiratory, and vascular access infections. SSI occur in 10 to 30% of cases after PD [ 9 , 10 ].…”
