Distribution of bony erosions in feet and performance of two bone erosion scores: A dual-energy computed tomography study of 61 patients with gout

Abstract: Objectives To assess the distribution of bone erosions and two erosion scores in the feet of patients with gout and analyze the association between erosion scores and monosodium urate (MSU) crystal deposition using dual-energy computed tomography (DECT). Materials and methods We included all patients who underwent DECT of both feet between 2016 and 2019 in our radiology department, with positive detection of MSU deposits. Data on sex, age, treatment, serum urate, and DECT urate volumes were obtained. CT imag… Show more

“…Therefore, there was no correlation between MSU crystals deposition, bone destruction, and serum uric levels. The relationship found in this study between serum uric acid and MSU crystal deposition is consistent with previous research ( 18 , 28 , 29 ). To dissolve MSU crystals and reduce clinical symptoms, guidelines worldwide consistently recommend the use of ULT to maintain serum uric acid levels ≤530 μmol/L ( 6 , 30 ).…”

“…The modified SvdH-CT erosion score was proposed in this study to detect the correlation between MSU crystal volume and erosion. This updated scoring method yielded performance consistent with that of the Rheumatoid Arthritis Magnetic Resonance Imaging Score system or Dalbeth-simplified score ( 29 , 36 ) and better performance than plain radiography erosion scores ( 16 , 36 ).…”

“…DECT has now emerged as a powerful tool to diagnose gout and detect MSU crystals deposition, while having the ability to initially assess bone erosion [9] . Recently, DECT has been found to be more accurate than ultrasound in detecting urate deposits outside the joints in soft tissues and some present in deep anatomical structure [10] .A prospective study showed that it also had the ability to detect intravascular MSU crystals deposition [11] , but it could not detect the presence of MSU crystals in the presence of liquid tophi and calci ed tophi [12] .…”
