Distribution of metastasis in the brain in relation to the hippocampus: a retrospective single-center analysis of 565 metastases in 116 patients

Abstract: ObjectiveTo examine the distribution of brain metastases (BM) in relation to the hippocampus, so as to determine the risk of metastasis in the hippocampus, and thus provide experimental evidence for the hippocampal avoidance (HA) in patients with BM during radiotherapy.Methods(1) For the retrospective analysis of 116 patients diagnosed with malignancies, confirmed as BM, from December 2014 to December 2016 at the First Affiliated Hospital of Bengbu Medical College. We obtained the T1-weighted, postcontrast axi… Show more

“…Hippocampus-avoidance WBRT (HA-WBRT) selectively restricts the radiation dose in the hippocampal region with the intention of preserving cognitive functions. It is generally considered a safe method, with a low risk of hippocampal and peri-hippocampal relapse (10,(14)(15)(16). The protective effect of HA-WBRT on the hippocampi has been and is currently still being investigated in prospective clinical trials, but mainly indirectly by means of neurocognitive testing.…”

“…According to the results of a study by Sun (2019) on 116 Chinese patients with brain metastases from multiple cancers, the metastasis rate in the hippocampus was 1.7%, and the metastasis rate in the hippocampus + 5 mm area was 11.2% (13). Based on data of 226 Chinese patients with brain metastases, Han reported that the metastasis rate in the hippocampus + 5 mm area was only 3.1%.…”

“…Yet, the safety of HA-WBRT depends on the incidence of brain metastases in the hippocampus and its surrounding areas. Studies have reported hippocampal metastasis rates of different tumors between 1.7% and 4.6%, and that of the hippocampal + 5 mm area between 3.1% to 12.2% (12)(13)(14)(15)(16). Nevertheless, the results in the existing studies vary considerably, and the applicability of HA-WBRT in patients with primary tumors such as SCLC or a large number of brain metastases is still controversial.…”

“…Related studies have shown that the metastatic involvement of the NSC regions (especially hippocampus) is unusual and the risk for the metastases within 15 mm of the hippocampus is lower than 15%. 19,20 According to statistics, only 2.7% of the metastases appeared within 15 mm of the hippocampus due to inadequate dosage after HA-PCI treatment. 21 Although the present study reserved a larger region (than 15-mm hippocampal expansion) for dose decline, the PTV_plan (PTV_plan1 + PTV_plan2) has been 2 has shown that the PD coverage of PTV-15mm was significantly higher for the improved VMAT plans than the other two plans.…”

“…If the PD covers nearly 100% volume of the remaining PTV (PTV_plan), the coverage of the whole brain PTV can easily meet the treatment prescription (V 25 Gy ≥ 90%). Related studies have shown that the metastatic involvement of the NSC regions (especially hippocampus) is unusual and the risk for the metastases within 15 mm of the hippocampus is lower than 15% 19,20 . According to statistics, only 2.7% of the metastases appeared within 15 mm of the hippocampus due to inadequate dosage after HA‐PCI treatment 21 .…”
