Do Intra-articular Corticosteroid Injections Prior to Total Knee Arthroplasty Increase Postoperative Complication Rates: A Retrospective Review

Abstract: There is conflicting literature suggesting that intra-articular corticosteroid injections before total knee arthroplasty (TKA) may lead to an increase in the rate of postoperative complications, specifically periprosthetic joint infection (PJI). Thus, this retrospective review of all TKAs performed at a large, urban hospital will add valuable evidence to help guide future patient care. After exclusion criteria, we retrospectively reviewed 417 patients who received a TKA from a group of fellowship-trained ortho… Show more

“…Patients receiving injections between 3 and 6 months prior to TKA had an overall infection rate of 2.60% (782/30,134). Control patients in these six analyses [2, 4, 6, 8, 34, 35] demonstrated a PJI rate of 2.29% (4024/175,930). There were no differences in infection rates following pooled analysis (OR: 0.82, 95% CI: 0.47–1.43; p = 0.49) (Fig.…”

“…Traditional total joint arthroplasty guidelines demarcate >3 months prior to surgery as the safe time interval to administer CSI. The literature reports varying time frames in which CSI may be safely administered prior to TKA, and the association between CSI and PJI has even been refuted by some 2,3,13,14 .…”
