Does caudal analgesia increase the rates of urethrocutaneous fistula formation after hypospadias repair? Systematic review and meta-analysis

Abstract: Introduction: Caudal block analgesia is administered to lower the requirements of systemic and inhaled anesthetic drugs during hypospadias surgery. However, recent reports, all clustered in a short time-span have generated controversial and mutually opposing results while evaluating caudal block as an independent risk factor for urethroplasty-related complications after hypospadias repair. There is no consensus statement on the role of caudal block analgesia in formation of urethrocutaneous fistul… Show more

“…Suture material, infection, previous operations, catheterization, surgical skills like tension-free anastomosis and prevention of epithelium eversion, all are implicated in prevention of fistula formation [ 6 , 13 ]. Another important factor in fistula prevention is the use of intermediate layer between neourethra and skin.…”

“…Kundra et al . [ 6 ] conducted a study to compare the analgesic effect of penile block vis-à-vis caudal epidural in hypospadias repair. They reported that all the urethra-cutaneous fistula (UCF) in the study cohort were observed in the caudal epidural group.…”
