Does Colchicine Substitute Corticosteroids in Treatment of Idiopathic and Viral Pediatric Pericarditis?

Abstract: Background and Objectives: Recurrence of pericarditis (ROP) is an important complication of the acute pericarditis. The aim of this study was to analyse the influence of aetiology, clinical findings and treatment on the outcome of acute pericarditis. Methods: Data were retrospectively collected from medical records of patients treated from 2011 to 2019 at a tertiary referent heart paediatric center. Results: Our investigation included 56 children with idiopathic and viral pericarditis. Relapse was registered i… Show more

“…Using polymerase chain reaction (PCR), pericarditis caused by a virus can be diagnosed. It can be treated with nonsteroidal anti-inflammatory drugs, steroidal medications, and colchicine [ 3 ]. According to the literature review, viral infections are linked to pericarditis, with coronavirus disease 2019 (COVID-19) and smallpox patients having a serious but uncommon incidence of pericarditis.…”

“…On the other hand, in addition to its marked anti-inflammatory and anti-fibrotic properties [ 12 ], colchicine has shown, at least in vitro, its effectiveness as an inhibitor of viral replication [ 13 15 ]. Treatment with colchicine has shown clinical efficacy in a small series of patients with severe myocarditis [ 16 ] and pericarditis of viral etiology [ 17 ]. In addition, viroporins are viral proteins that alter the permeability of the membrane and can trigger the activation of the inflammatory cascade [ 18 ].…”
