Does Lithium Treatment Still Work?

Abstract: To evaluate whether lithium treatment has been overvalued and may be no longer as effective as formerly, we reviewed published reports on long-term lithium treatment (1970-1996) as well as analyzing its clinical effects on 360 patients with DSM-IV bipolar disorder who entered into lithium maintenance monotherapy after 1970. Neither reported recurrence rates nor average proportions of time ill nor patient improvement of 50% or more during lithium maintenance therapy in a stable clinic setting has changed signif… Show more

“…Besides the broadening of diagnostic criteria, this might be attributed to a selection bias that results from the fact that the patients were recruited in university hospitals. Patients with more atypical, treatment-unresponsive illness may be overrepresented in most research centers [8,53]. In general, the use of lithium is limited by its narrow therapeutic range and its disturbing side effects [54,55].…”

“…Their main advantage usually is the length of observation, in some instances reaching several decades 363, 364, 365. This comes at the cost of generalizability, in terms of both patient selection and non‐random treatment allocation.…”

“…It is also important to acknowledge that lithium is stably effective in patients with typical BD, and that the more recent reports of reduced lithium efficacy could be attributed to expansion of the diagnostic criteria of BD in DSM-based systems. 2427 Long term studies indicate that the distribution of response to lithium is typically bimodal, sometimes trimodal, with a smaller group of partial responders. 28 It is unclear whether partial responders are similar to responders, except that their treatment may not be optimized with respect to plasma levels or there is inadequate attention to adherence to treatment.…”
