OBJECTIVETo validate the Sexual Assessment Monitor (SAM), a novel apparatus designed to collect electronic data on ejaculatory latency time (ELT) for diagnosing premature ejaculation (PE), and for accurately measuring treatment outcomes in clinical trials. PATIENTS, SUBJECTS AND METHODSMen with PE, and healthy volunteers aged 18-75 years, were enrolled in three openlabel studies, conducted in the UK. The SAM, which consists of a control box with two front attachments, a vibrator and sensor, was attached to the penis. The vibrator, which provides stimulation, was positioned at the frenulum using a soft cuff; the vibrator intensity was set at 80 units for most subjects. The sensor is an indiumgallium elasticated loop, which was positioned around the base of the penis to detect ejaculatory pulses. These pulses were transmitted to a data recorder in the control box. The data, which are displayed graphically as traces, were automatically classified by a computer-generated algorithm to quantify ELT. RESULTSIn all, 53 healthy volunteers and 58 men with PE provided 213 and 195 evaluable records, respectively. Most were complete records (99% and 96%). The pooled data showed that the ELT was much higher for healthy volunteers than for men with PE (geometric means: 687 vs 169 s, respectively), with a healthy volunteer to PE patient ratio of 2.87 ( P < 0.001). Only 6.3% of subjects reported mild adverse events, which were unrelated to the SAM. CONCLUSIONSThese open-label studies show that the SAM can consistently and safely measure times to erection (from the start of vibration) and ejaculation, and ELT in healthy volunteers and men with PE. These findings show that the SAM has the potential to become the 'gold standard' in the diagnosis of PE and in clinical trials design.

Background: The use of erotic media is a controversial issue in the literature and for the general population. There seems to be no consensus on whether pornography use negatively impacts the sexual health. Aims: To raise the empirical and scientific bases of the impact of pornography consumption on human sexual function. Method: Systematic review of Pubmed, Scielo and BVS databases using the term 'pornography', restricted to clinical trials, case reports, incidence, prevalence, exploratory and risk factor studies. Excluded those who used pornography only as a tool to provoke sexual arousal in tests, those who did not describe the effects of using pornography itself, articles without available abstracts or not yet published and duplicated in the databases. Results: Of 24 resulting studies, only 15 crossed the two variables: 10 correlated pornography with better sexual function scores and 5 with worse. Conclusion: If pornography consumption has an impact on sexual function, it is positive, not negative. The pornographic market presents extremely serious problems, but the negative impact on people's sexual function is not one of them.

Introduction The Committee on “Clinical Evaluation and Scales in Sexual Medicine” of the third International Consultation in Sexual Medicine reviewed current practice and new developments in the field of physiological testing in male erectile dysfunction (ED). Aim To provide an overview of current practice and new developments in the field of diagnostic testing in male ED. Methods The Pubmed literature was reviewed. Results Since the 1980s of the last century, a broad array of specialized physiological tests has been used for assessing ED. The notion that ED often is an (early) symptom of generalized cardiovascular disease and the introduction of oral pharmacological therapies that are effective irrespective of etiology has reduced the application of the “classical” tests to a minimum and has shifted the scope toward tests with demonstrated reliability in cardiovascular medicine. Conclusions (i) The clinical utility of specialized tests in the evaluation of male ED is limited to a small minority of men; (ii) the scope of physiological testing has shifted toward tests with demonstrated reliability in cardiovascular medicine.
