Does vigorous exercise have a neuroprotective effect in Parkinson disease?

Abstract: Parkinson disease (PD) is progressive, with dementia and medication-refractory motor problems common reasons for late-stage nursing-home placement. Increasing evidence suggests that ongoing vigorous exercise/physical fitness may favorably influence this progression. Parkinsonian animal models reveal exercise-related protection from dopaminergic neurotoxins, apparently mediated by brain neurotrophic factors and neuroplasticity (predicted from in vitro studies). Similarly, exercise consistently improves cognitio… Show more

“…Given that time is perceived to be one of the main barriers to regular physical activity [9,10], vigorous exercise has a clear advantage over moderate-intensity exercise as a way to meet the current physical activity guidelines. There is also growing physiological and epidemiological evidence that vigorous exercise may be essential to maximise the benefits of physical activity [15][16][17][18]. Therefore, I believe it is worthwhile to find ways to facilitate vigorous exercise in the general population so that it is not limited to relatively few athletes and fitness enthusiasts.…”

“…Prevention:Exercise in the middle age reduces the risk of Parkinson's disease later in life [71].Caffeine also appears protective with greater decrease in risk occurring with larger intake of caffeinated beverages such as coffee [72].Although tobacco smoke causes adverse health effects, decreases life expectancy and quality of life, it may reduce the risk of PD a third when compared to nonsmokers[11t].The basis of this effect is not known, but possibilities include as effect of nicotine as a dopamine stimulant [11].Antioxidants, such as vitamins C and D, have been proposed to protect against the disease, but results of studies have been contradictory and no positive effect has been proven [11].Also there have been preliminary indications of possible protective role of estrogens and anti-inflammatory drugs [11].…”

“…Moreover, emerging literature suggests that staying physically active may actually have a favorable influence on brain integrity and neuroplasticity, possibly neuroprotective in PD. 17 5. geNeric immeDiAte-releAse 25/100 cArbiDoPA/ levoDoPA is AN excelleNt choice Carbidopa/levodopa dosage is given as 2 numbers, with the first representing the carbidopa component. Carbidopa protects against nausea, and hence formulations with more carbidopa are usually preferable and only slightly more expensive (ie, 25/100 vs 10/100).…”
