Dome Fracture of the Talus

Abstract: 1. Ten consecutive fractures of the dome of the talus are reported. Six were lateral, three medial and one anterior. 2. The injury is most commonly seen in the second and third decades of life, and with the exception of the rare anterior dome fracture, results from an inversion injury of the ankle. The possibility of a dome fracture associated with strain of the lateral ligament of the ankle joint should therefore always be borne in mind. 3. Antero-posterior views in neutral and in plantar fiexion are helpfu… Show more

“…A similar mechanism with regard to talar fracture exists in the literature regarding long falls with regard to the so called "dome fracture". (Mukherjee and Young, 1973) The load could then be passed largely to the sustentaculum which may fail allowing for the full fracture complex to develop. Such a path would also potentially explain why both in this series and in the long fall case described by Buschmann and Tsokos (2011), penetration of the fractured bone through the soles of the feet was noted to be on the medial aspect of foot in cases where such details were noted.…”

“…(1922). Since that time the importance of trauma as an aetiological factor was stressed and appreciated (Mukherjee and Young, 1973;Rendu, 1932;Roden et al, 1953;Yvars, 1976). It was not before the clinical and experimental observation of Berndt and Harty (1959) that the aetiology, classification and natural history of the lesion were fully understood.…”

“…Lesions of a more severe degree present persistent symptoms of pain and instability of the ankle (Alexander and Lictman, 1980;Mukherjee and Young, 1973). When untreated the chondral fracture becomes a loose fragment which destroys the articular cartilage and originates in early degenerative arthritis (Smith et al, 1977).…”

“…Osteochondral lesions of the talus (OLT) are a relatively common cause of chronic post traumatic ankle pain that can pose a diagnostic and management challenge. Many current studies are based on plain X-rays 2,4,11,12,13 and include all types of lesions. It is now clear that plain X-ray is inadequate when assessing OLT.…”
