Dopaminergic resistance in a case of invasive macroprolactinoma

Abstract: Invasive prolactinoma is a relatively infrequent variety of macroprolactinoma characterized by a fast and aggressive growth, with infiltration to adjacent structures, and whose management is frequently difficult. We present the case of a fatal invasive macroprolactinoma in whom resistance to different dopaminergic drugs developed.

“…3) Fibrous change occurs after administration of BC for more than 3 months, which makes surgical intervention difficult. 4) BC resistant prolactinomas are encountered in about 10% of cases [4,6,8,10,11]. 5) Even though the total removal is not achieved, the dose of BC can be decreased by surgically lowering the PRL level.…”

“…Several cases have been reported in which prolactinomas failed to respond to dopamine agonist medication, [2,4,6,9,11] but, criteria for recognizing dopamine agonist resistance have not been clearly formulated, since patients were treated with different doses [6,10]. Furthermore the duration of treatment has not been the same and the evaluation has not been uniform.…”
