Eighteen patients who referred to our hospital with acute scrotal pain and swelling were evaluated with color-Doppler imaging (CDI) after the initial clinical examinations. Five of these subjects were considered to have testicular torsion as diagnosed with CDI. These diagnoses were surgically confirmed, and appropriate treatments were performed. CDI findings of 13 patients were in accordance with inflammatory pathologies (epididymitis and/or orchitis). These patients received medical treatment. The CDI diagnoses of all patients were in agreement with the final clinical diagnoses. We conclude that CDI, which is a practical, rapid and high-resolution technique, promises to be the leading and decisive scanning method in acute scrotal disorders.

RESUMEN TORSIÓN TESTICULAR EN EL PACIENTE GERIÁTRICO. Y REVISIÓN DE LA LITERATURA:A PROPÓSITO DE UN CASO La torsión testicular en el adulto, es una nosología infrecuente aunque debe de entrar dentro del diagnostico diferencial de cualquier adulto de mas de treinta años, con cuadro de escroto agudo. El retraso e incorrecto diagnóstico, aumenta la frecuencia de orquiectomías por procesos vasculares en el adulto frente al niño. Presentamos el caso de varón de 74 años con escroto agudo, y revisión de la literatura. N NO OT TA A C CL LÍ ÍN NI IC CA A L a torsión testicular es una afectación frecuente en la infancia, cuya incidencia es de uno por cada 4000 varones. Varía según los autores teniendo dos picos máximos a los 8 y 13 años, disminuyendo progresivamente según avanza la edad. Conforme el hombre va envejeciendo, la etiología del escroto agudo va siendo mayormente infecciosa (orquitis y orquioepididimitis) e infrecuentemente vascular. Aunque es anecdóti-co, la torsión testicular en el adulto, existe; por lo tanto siempre debe formar parte del diagnostico diferencial del escroto agudo en mayores de 30 años. In the acute scrotum, diagnosis can be difficult in certain cases. In 107 patients the value of static and dynamic radionuclide imaging was evaluated. Static images provide a sensitivity and reliability of over 90%. Quantitative measurements do not give additional information in the differential diagnosis of these patients. Generally, scintigraphy can only be advocated if available as an emergency procedure. It is not necessary in cases with clinically obvious diagnosis of acute testicular torsion or epididymitis, but can be very helpful and reliable in unclear cases, i.e. for the evaluation of testicular perfusion after scrotal trauma.
