Doppler Ultrasound Versus Testicular Scanning in Theevaluation of the Acute Scrotum

Abstract: A comparative prospective study of the Doppler ultrasonogram, the testicular scan and the surgical findings in patients with an acute scrotum is presented. The testicular scan was found to be the most reliable diagnostic method since it correlated with the surgical findings in 100 per cent of the cases. The Doppler ultrasonogram proved to be useful since it gave a positive correlation in 79 per cent of the cases studied.

“…Continuouswave Doppler examination, with its inability to localize flow precisely, is not acceptable as a routine imaging tech nique for acute scrotal disorders [5,[9][10][11][12], Conventional duplex Doppler studies did not increase the accuracy of sonographic evaluation of acute scrotum to a significant extent. The reason is the difficulty in detecting testicular vessels, which are small in size and have low-velocity flow.…”

“…Radionuclide scanning was reported in various studies [2,5] to have an accuracy rate of 89-100% in diagnosing testicular torsion. Although it demonstrates the perfusion status of the testis, its resolution is poor, and paratesticular vessels may cause false-negative results.…”

“…Doppler sonography can be unreliable under these conditions. The increased perfu sion of the scrotal wall may mimick a positive blood flow in the testicular artery and false-positive results are pos sible even when applying the funicular compression test described by Pedersen et al [20], Therefore, the overall rate of false-positive or false-negative results can be rela tively high [3,22]. Sonography allows visualization of the morphological changes, but it is not suitable for the eval- uation of functional disturbances such as testicular tor sion [4,13,14,21], Scintigraphy proves, as other authors described before [5-7, 12, 25], to be a very reliable and sensitive as well as specific diagnostic procedure.…”

“…In some investigations it had, however, a rate of false-posi tive or false-negative results of up to 20% [3,22], Sono graphy is very reliable in the diagnosis of testicular tumors and is a very sensitive, but unspecific investigation in respect to morphological changes. It is, however, an un suitable method to diagnose acute testicular torsion [4,13,14,21] and other functional disorders.…”
