Dorzolamide-timolol combination and retinal vessel oxygen saturation in patients with glaucoma or ocular hypertension

Abstract: Adding dorzolamide to timolol monotherapy has a minimal effect, but going from dorzolamide-timolol combination to timolol alone lowered arteriolar and venular oxygen saturation. The retinal oxygen saturation measurements show a high degree of stability over an extended period of time. Previous studies have suggested increased retinal and optic nerve blood flow with dorzolamide. Unchanged oxygen saturation and increased blood flow would indicate increased oxygen delivery to the retina.

“…Previous literature has suggested the positive effect of IOP control on improved ocular blood flow and hence retinal oxygen saturation. 24,25 Further studies involving blood flow measurements and a larger sample size would be required for specific assessment of retinal SO 2 under different therapeutic options.…”

“…Diseases investigated so far include diabetic retinopathy (Hammer et al, 2009, Hardarson andStefansson, 2012b), glaucoma , Traustason et al, 2009) and retinal vein occlusion Stefansson, 2010, Hardarson andStefansson, 2012a). Ease of use and low variability paired with high sensitivity and specificity are essential for any diagnostic technology.…”

“…This improvement in blood flow has also been reported in retinal arteries, with dorzolamide accelerating the arteriovenous passage of fluorescein dye when compared with a beta-blocker . Topical carbonic anhydrase inhibitors may also play a role in retinal oxygenation because withdrawal of the drug leads to a decrease in arterial and venous saturation (Traustason, 2009). However, this class of drug is mostly used in association with other drugs, namely, beta-blockers.…”
