Dosimetric Comparison of Helical Tomotherapy and Intensity-Modulated Proton Therapy in Hippocampus- and Scalp-Sparing Whole Brain Radiotherapy

Abstract: Objective: Cognitive decline and alopecia after radiotherapy are challenging problems. We aimed to compare whole brain radiotherapy (WBRT) plans reducing radiation dose to the hippocampus and scalp between helical tomotherapy (HT) and intensity-modulated proton therapy (IMPT). Methods: We conducted a planning study of WBRT for 10 patients. The clinical target volume was defined as the whole brain excluding the hippocampus avoidance (HA) region. The prescribed dose was 30 Gy in 10 fractions to cover 95% of the … Show more

“…According to few studies, IMRT methods can achieve simultaneously scalp- and hippocampus-sparing. 13,14 For the future cohort study, it would be crucial to incorporate hippocampus-sparing into scalp-sparing WBRT plans for minimizing scalp and neurocognitive side-effects.…”

“…Takaoka et al. ( 43 ) compared WBRT plans between Tomo and IMPT, highlighting IMPT’s superior target coverage over Tomo and its significant reduction in the dose received by the hippocampus compared to the scalp. Consequently, HA-WBRT using the IMPT technique proves to be an effective treatment option for preventing cognitive decline and hair loss.…”

“…Currently, there are various modalities available for whole-brain hippocampus-avoidance, including intensity-modulated radiation therapy (IMRT) ( 9 ), volumetric modulated arc therapy (VMAT) ( 10 , 11 ), and tomotherapy (TOMO) ( 12 , 13 ), that have been used in clinical practice to achieve hippocampal sparing during brain metastases treatment. In order to offer further protection to the hippocampi and decrease radiation dose to critical organs, several scholars have introduced a positioning method termed “head tilt,” which involves tilting the head at a particular angle ( 14 19 ).…”
