Drug-eluting stents for the percutaneous treatment of the anastomosis of the left internal mammary graft to left anterior descending artery

Abstract: PCI of the stenoses of LIMA-to-LAD anastomoses with DES did not provide any clinical improvement over BMS use in long-term outcomes; DES use was associated with some cases of late thrombosis.

“…Knowledge available in the literature on the treatment of LAD stenosis located distally to its anastomosis with the LIMA or including the site of anastomosis is scarce and ambiguous. Attempts are being made to treat these lesions percutaneously or surgically, but none of these strategies has shown a clear advantage, and anatomical diversity implies the need to make individual decisions [24]. In the present case, the PCI procedure was fully successful as demonstrated angiographically and clinically.…”

“…Arterial graft dysfunction is usually caused by general atherosclerotic disease progression and neointimal hyperplasia caused by vascular trauma during CABG [ 90 ]. When comparing DES with BMS for percutaneous revascularization of internal mammary artery grafts, no statistically significant difference was found in the rates of TLR in several studies [ 91 , 92 ]. Patients with a history of CABG have a higher risk of cardiac death and TVR at 5-year follow-up, even if newer generation DES are used [ 93 ].…”

“…Причиной этого является повреждение эндотелия вен в условиях артериальной гемодинамики, к которой вены не приспособлены. В результате происходит пролиферация гладкомышечных клеток (ГМК), ведущая к гиперплазии неоинтимы [17,19,20]. Другим механизмом облитерации венозных кондуитов является очаговая лейкоцитарная инфильтрация эндотелия вен, что также способствует потере их просвета [21].…”
