Drug-Induced Liver Injury Associated with Statins

Abstract: The "statins," or hydroxymethylglutaryl coenzyme A (HMG-CoA)-reductase inhibitors, are a generally safe class of drugs that are widely used throughout the world and are rarely associated with severe hepatotoxicity. In this article, two cases of severe hepatotoxicity attributed to statin use are presented. In addition, a detailed summary of previously published cases of statin hepatotoxicity and the risks and benefits of statins in patients with chronic liver disease are presented. Drug-induced liver injury (DI… Show more

“…New drugs: the growing use of drugs such as statins and biologic agents, which have been related to DILI with autoimmune features, which has prompted an increase in the diagnosis of DIAILD [36][37][38][39][40][41][42] . The diagnosis DIAILD represents a challenge to the clinician as there are neither histological, nor clinical features that are pathognomonic of true AIH, and HLA haplotypes do not convincingly distinguish between either entity.…”

“…However, numerous clinical cases have been published reporting the various ADR induced by statins [18,19]. These ADRs include liver damage, myopathy and rhabdomyolysis, myalgias and polyneuropathy [20].…”

“…Despite their widespread use, acute liver failure and death have rarely been reported in patients with statin hepatotoxicity. Multiple retrospective studies, as well as one large prospective randomised controlled trial demonstrate that statins can safely be given to hyperlipidaemic patients with compensated chronic liver disease (49).…”
