Drug-induced thrombocytopenia in children

Abstract: Drugs and other substances must be considered as potential causes of thrombocytopenia. Evidence from published reports and data for drug-dependent, platelet-reactive antibodies can help clinicians evaluate of children with unexpected thrombocytopenia.

“…Secondary IT can be caused by the assumption of drugs and vaccines: drug-induced IT, ascribable in most of cases to the assumption of certain antibiotics, non-steroidal anti-inflammatory drugs, and antivirals, it is often not recognized, resulting in recurrent non-explained episodes of thrombocytopenia ( 21 ), that usually show a complete recovery after the withdrawal of the drug. Less commonly than in adulthood, it is possible to observe in childhood the development of heparin-induced thrombocytopenia ( 22 ).…”

“…Immune thrombocytopenias are among the most important causes of acquired thrombocytopenias. Probable or definite drug-related thrombocytopenia can be induced by several drugs and may occur because of both decreased production and/or peripheral destruction of thrombocytes [1]. Polymyxins are effective antibiotics for resistant Gram-negative bacteria with a potential adverse effects of nephrotoxicity and neurotoxicity which are often dosedependent and reversible on discontinuation of treatment [2].…”

“…In suspected drug-related thrombocytopenia other conditions that can cause thrombocytopenia must be excluded [1]. Drugrelated thrombocytopenia can be mediated by several drugs and may occur because of both decreased production and/or peripheral destruction of thrombocytes [1,3]. Immune thrombocytopenias are the most important reasons of acquired thrombocytopenias.…”

“…The clinical presentation of both diseases might be similar, but the time course and resolution upon drug discontinuation can distinguish PIPT from DIPT. Drugs most associated with DIPT in children include acetaminophen, carbamazepine, ceftriaxone, phenytoin, quinine, sulfasalazine, and co‐trimoxazole …”
