Drug points: Neonatal convulsions after withdrawal of baclofen

Abstract: Baclofen is a skeletal muscle relaxant used for the relief of chronic severe spasticity resulting from disorders such as multiple sclerosis or traumatic injury to the spinal cord. We report convulsions in a 7 day old girl who had been exposed to baclofen during intrauterine life. A paraplegic mother had been taking baclofen 20 mg four times daily (Lioresal, Novartis, Surrey), oxybutanin 3 mg three times daily, and trimethoprim 100 mg daily, which she continued throughout her pregnancy. The pregnancy was uneven… Show more

“…Baclofen should be used with caution during pregnancy: although there are no reports of baclofen directly causing human fetal malformations, animal studies using high doses show that it causes impaired sternal ossification and omphalocele 24. Withdrawal from maternal baclofen occasionally provokes neonatal convulsions25 and so newborns of mothers taking baclofen should receive oral baclofen 0.5 mg per kg per day, weaned off over 9 days 26. Baclofen appears in breast milk and this must be considered when advising mothers about breast feeding.…”

“…The symptoms of withdrawal are most often limited to return of the patient's baseline rigidity [8]. In more severe cases, symptoms may include seizures, acute psychosis, delirium, fever, labile blood pressure or hypotension and lowering of consciousness [56,57,58,59,60,61,62,63]. These symptoms usually improve after the reintroduction of baclofen; other treatment options may be benzodiazepines, propofol, skeletal muscle relaxants and tizanidine [61].…”

“…In two cases, 20-80 mg was taken orally throughout the whole pregnancy. Again there were no malformations, but both children suffered from withdrawal -one with seizures on day 7 (Ratnayaka 2001). The other newborn showed hyperirritability and respiratory problems (Moran 2004).…”
