Navade, povezane z zdravjem in socialno demografske značilnosti nosečnic v Sloveniji tina kek,1 nataša karas kuželički,2 Irena Mlinarič raščan,2 ksenija Geršak 1,3 IzvlečekIzhodišča: Nosečnost je obdobje, ko ženske za zdravje bodočega otroka kritično presojajo in ocenjujejo svoje zdravje ter z zdravjem povezane navade. Skupaj z zdravstvenimi delavci se srečujejo s šte-vilnimi tveganji in odločitvami, ki postanejo veliko kompleksnejše, saj je v igri vpliv na dva organizma. Namen članka je pregled ter iskanje morebitnih povezav med zdravjem, z zdravjem povezanimi navadami ter socialno demografskimi dejavniki v nosečnosti, ki so jih poročale sodelujoče ženske.Metode: Podatke študije smo pridobili v okviru raziskovalnega projekta Analiza bioloških označe-valcev presnove folatov pri ugotavljanju tveganja za nastanek napak nevralne cevi, orofacialnih shiz ter prirojenih srčnih napak, ki je potekal od maja 2013 do septembra 2015. Vprašalnik o maternalnem zdravju, z zdravjem povezanih navadah ter socialno demografskih karakteristikah je izpolnilo 450 sodelujočih žensk. Podatki zajemajo nosečnosti, ki datirajo od osemdesetih let prejšnjega stoletja pa do leta 2015.Rezultati: Mlajše in manj izobražene ženske so pogosteje poročale o kajenju med nosečnostjo, medtem ko so starejše, bolj izobražene ter nulipare ženske v višjem odstotku jemale folatne/multivitaminske dodatke. Opazili smo U-obliko porazdelitve uporabe zdravil (brez recepta in na recept) glede na stopnjo izobrazbe. Najvišji odstotek uporabe so imele ženske z najvišjo stopnjo izobrazbe (magisterij/doktorat) in ženske z doseženo najnižjo stopnjo izobrazbe (osnovna šola). Večjo uporabo zdravil med nosečnostjo so poročale tudi starejše ženske. Opazili smo trend višanja uporabe zdravil in folatnih/multivitaminskih pripravkov kot tudi incidence nosečniške sladkorne bolezni preko preiskovanega obdobja, z najvišjo incidenco v zadnjih desetletjih. Višjo pojavnost kroničnih bolezni smo opazili v skupini multiparih žensk.Zaključki: V navadah povezanih z zdravjem nosečnic obstajajo precejšnje socialno demografske razlike. Za izboljšanje zdravja so potrebni izboljšani javno zdravstveni ukrepi in individualno svetovanje, da lahko ustrezno naslovimo specifične potrebe socialno demografskih skupin z višjim tveganjem za neželene izide nosečnosti. Background Because of the limited data concerning drug risks in pregnancy, health professionals are often deprived of relevant and sufficient information related to prescribing or dispensing during pregnancy. However, previous studies have emphasised the widespread French prescription of several drugs (sometimes "typically French") which have not been assessed in pregnant women. Objectives The aim of the present study was to create the first French database of drugs prescribed and dispensed during pregnancy and the outcome of these pregnancies. Methods This feasibility study concerns pregnant women who gave birth to a baby between 1 July 2004 to 30 June 2005 in Haute-Garonne and who are registered in the French Health Insurance Service. Data sources include (1) the French Health Insurance Database (drugs prescribed during pregnancy), (2) the Mother and Child Protection Centre Database (newborn health at birth and 9 months after) and (3) the Antenatal Diagnostic Centre Database (medical pregnancy interruptions). Results The database is composed of 10,174 "motheroutcome" pairs. The prevalence rate of congenital anomalies was 2.2%. Pregnant women were prescribed 11.3±8.2 different drugs. Among the 20 most frequently prescribed drugs, around half of them have not been evaluated in pregnant women. Conclusions The first results of this study show that implementation of a French database on prescription of drugs and pregnancy outcomes is feasible. Compared with several databases available in other countries, EFEMERIS provides exact data on period of exposure to drugs, pregnancy terminations, and follow up of the baby 9 months after birth. Recording these data would make it possible to assess the risk of malformations due Eur J Clin Pharmacol (2009) to a greater number of drugs and would contribute to international drug evaluation studies.

Saúde Pública 2017; 33(2):e00126215 introdução A utilização de medicamentos por gestantes passou a ser pauta de uma grande discussão a partir da década de 1950, com a tragédia da talidomida, um medicamento sedativo utilizado no tratamento de náuseas e vômitos durante a gravidez, que levou ao nascimento de cerca de 10 mil crianças com focomelia, uma má-formação congênita, que até aquele momento era considerada rara 1 .Antes desse episódio, acreditava-se que a placenta era uma barreira que protegia o feto de qualquer ação farmacológica; hoje, porém, sabe-se que a maioria dos fármacos utilizados por gestantes atravessam a placenta e atingem a corrente sanguínea do feto 2,3 , que, não possuindo os sistemas corporais plenamente desenvolvidos, está mais sujeito a efeitos negativos não esperados 4 .A não inclusão de mulheres em idade fértil nos ensaios clínicos de novos fármacos, bem como as evidências de que os estudos de toxicidade em animais não preveem com segurança os efeitos em humanos, justificam os cuidados que se deve ter com o uso de medicamentos em gestantes 5 . No entanto, dado que nas sociedades modernas o uso de medicamentos tornou-se prática costumeira e de dimensão crescente, a exposição de conceptos em períodos iniciais da gestação é um evento de possibilidade frequente, especialmente em gravidezes não planejadas 6 . Tais fatores ressaltam a importância da realização de estudos observacionais sobre o uso de medicamentos na gravidez, que têm se intensificado nas últimas décadas 6 .Conforme esses estudos, as prevalências de uso de medicamentos antes da gestação em países europeus variam de 29,9% a 43%, sendo os grupos terapêuticos mais utilizados os antibacterianos de uso sistêmico, hormônios sexuais e anti-inflamatórios não esteroides 6,7,8,9,10 . Investigação realizada em cidade do Sul do Brasil mostrou que 46,7% das mulheres estavam em uso de medicamentos no momento do diagnóstico da gravidez. O percentual mais elevado de automedicação ocorreu antes do início do pré-natal (64,9%) 11 .Poucos trabalhos investigaram a associação entre os medicamentos utilizados antes da gestação e características socioeconômicas da mulher. Foi encontrado apenas um estudo sérvio em que idade materna (≥ 30 anos), renda familiar per capita (> 300 Euros) e parição (mais de 1 filho) estiveram entre os fatores associados 12 .Quanto ao uso de medicamentos durante a gestação, em países como França, Canadá, Estados Unidos e Brasil, foi verificado que mais de 80% das mulheres utilizam algum tipo de medicamento nesse período 13,14,15,16 , ent...
