Dual intraocular lens implantation: Monofocal lens in the bag and additional diffractive multifocal lens in the sulcus

Abstract: Combined implantation of an add-on diffractive sulcus IOL and a monofocal capsular bag IOL was safe and effective in improving far and near visual acuity in cataract surgery. Preliminary visual acuity results were similar to those in eyes with a single 1-piece diffractive multifocal IOL.

“…[1][2][3][4] To our knowledge, this clinical case represents the first implantation of a combined toric diffractive-additive IOL. It shows that the addition of a cylinder correction to a multifocal optic is also possible in additive IOLs, which increases the number of selectable patients for additive IOL implantation.…”

“…They can be implanted in the sulcus of pseudophakic eyes to correct residual refractive errors after cataract or refractive surgery and are also a good option in cases of dynamic refraction changes since they can be easily explanted or exchanged. [1][2][3][4] The convex-concave optic shape of these IOLs prevents contact with the primary IOL in the capsular bag.…”

“…Add-on IOLs are now available not only to correct spherical errors but also to correct astigmatism using toric designs and to provide near visual acuity without glasses using multifocal designs. [12][13][14] These IOLs have become part of refractive surgery and are accompanied by high patient expectations. To meet these expectations, correct IOL power calculation, IOL rotational stability, and a predictable IOL position without unexpected vault are necessary.…”
